QHAH welcomes residential referrals for adults with developmental disabilities. We work with individuals, support coordinators, Community Services Boards, families, guardians, hospitals, REACH, case managers, and other authorized referral sources.
Our referral process helps us understand the person, the support they need, and whether QHAH may be an appropriate residential provider.
How It Works
Start the Referral

Submit the Referral
Complete ADM-001 and contact QHAH for secure submission instructions.
QHAH Reviews the Referral
We review the information and follow up if anything else is needed. Next steps may include records review, assessment, meeting, or tour.

ADM-001 is intended for CSBs, support coordinators, case managers, hospitals, REACH, and other authorized referral sources seeking developmental disability group home residential services.
